Facilities Participation Agreement

To request participation for your facility, use our contact form below to reach out and start the enrollment process.
Once we have confirmed your facility is eligible to participate, an invitation email will be sent to the authorized signer for the facility to sign the participation agreement. Nevada Rural Hospital Partners will review and countersign the participation agreement. You will receive a signed copy for your records.
- Banking information is required for electronic funds transfer (EFT) to your account.
- A copy of your Nurse Apprentice Program policy is required to be uploaded as part of the participation agreement.

Eligible facilities for the Nurse Apprentice Program ARPA Grant are limited to the following:
- Critical Access Hospitals
- Long-Term Acute Care Hospitals
- Non-Profit Hospitals
- Skilled Nursing Facilities
- State Facilities

Each month, your facility must submit a payment request form. You will be asked to upload your Nurse Apprentice’s timesheet. Please note that this form must be submitted each month for each nurse apprentice. Requests are due by the 10th of each month for the hours worked in the prior month.
For facilities that have both Skilled Nursing Facility (SNF) and Acute departments, we require that you report monthly hours worked in each department.

9/20/22 NOTICE: LPNs are paid at a different rate so a separate form (above) must be completed for LPN Nurse Apprentices.

Lets keep our nurses in Nevada
If your facility has offered a retention bonus to a former nurse apprentice who is now licensed as a nurse, you can request a retention payment of up to $4,000 here. Proof of licensure, a copy of the signed agreement between the facility and nurse, with a copy of the check showing the bonus was paid is required.
